Does your education program require hands-on experience under the guidance of an experienced professional? Whether you’re pursuing a clinical or non-clinical profession, there is an opportunity for you!

Our Education Placement Team facilitates a variety of student opportunities, including clinical placements and preceptorships, with guidance and supervision from experienced health care professionals.

Agencies (learning institutions, employers, associations/licensing bodies, or community organizations) must submit a placement request through the Horizon Education Placement Portal at least three months in advance of the placement start date.

Who Can Request a Placement?

Horizon education placements must be requested by a learning institution, association/licensing body, employer, community organization, or agency on behalf of the student/learner or employee.

Requests submitted directly by students will not be accepted.

Learning Institutions

An accredited, recognized, or approved learning institution (or agency) representing their student who requires an education placement to meet curriculum requirements.

Associations

An association/licensing body (or agency) representing their member (or learner) who requires an education placement to fulfill certification or recertification requirements.

Employers

An employer (or agency) representing their employee (or learner) who requires an education placement to maintain or develop clinical competencies.

Organizations

A community organization (or agency) representing youth/adults (or learners) who participate in group and individual academic training, work readiness training, and life coaching in preparation to either return to school or enter the workforce.

Affiliation Agreement

All placements are governed by an Affiliation Agreement, which outlines roles, responsibilities, and legal obligations between Horizon and the requestor. An Affiliation Agreement must be in place before the agency submits an education placement request.

Frequently Asked Questions

The Affiliation Agreement is a legal contract between Horizon Health Network and the Agency, signed by authorized
representatives from both parties. It outlines the roles and responsibilities of each party in facilitating education
placements. While the Agreement does not guarantee placement opportunities, it clearly defines the obligations of
each party. Only Horizon’s official template will be used to support education placement experiences hosted by
Horizon.

The Horizon Health Network Affiliation Agreement includes the following key sections:

  • Definitions and Terminology – Key terms and definitions used in the agreement.
  • Conditions of Affiliation – Requirements that both parties must adhere to.
  • Agency and Agency Coordinator Responsibilities – Commitments and obligations the agency and its appointed
    coordinator must uphold under the agreement.
  • Horizon Responsibilities – Responsibilities of Horizon Health personnel.
  • Privacy and Protection of Personal Information – Provisions ensuring confidentiality and data protection.
  • Insurance and Indemnification – Coverage and liability provisions.
  • Dispute Resolution – Procedures for addressing and resolving disputes between the parties.

Horizon Health Network determines which agencies and education programs qualify for placements. Having an
affiliation agreement does not guarantee automatic approval for all programs; each program requires separate
approval.
